Currently, the two main types of implant fills in use are silicone gel and saline. Silicone implants are filled with a highly-cohesive gel, often referred to as “gummy bear” filling, which maintains its shape even if the shell of the implant is compromised. On the other hand, saline implants are filled with a sterile saline solution at the time of surgery.

So, which is better, silicone or saline? Each type has its own set of advantages and disadvantages. Silicone implants, for instance, are favored for their natural look and feel, and they tend to have less rippling compared to saline implants. Additionally, the cohesive gel in silicone implants offers a more stable form. Conversely, saline implants offer the benefit that if they rupture, the body naturally absorbs the saline solution. It's worth noting that silicone implants are typically reserved for patients over the age of 22 for cosmetic procedures, although exceptions may be made for corrective surgeries.

As for the placement of the implants within the breast, there are various options tailored to individual needs. They can be positioned either directly under the breast tissue and on top of the pectoralis muscle (sub-glandular plane) or beneath both the breast tissue and the pectoralis muscle (sub-pectoral plane). In some cases, a combination of these placements, known as a dual-plane pocket, may be recommended.

The surgical procedure typically lasts around 1.5 hours and is performed under anesthesia to ensure your comfort throughout.

Recovery from breast augmentation varies from person to person, but most patients experience chest soreness that can persist for several weeks, particularly if the implants are placed beneath the pectoralis muscle. During the initial recovery period, wearing a supportive sports bra and a bandeau or strap is recommended to help the implants settle into their proper position on the chest wall. This supportive wear is typically worn continuously for the first 3-4 weeks post-surgery.

In terms of physical activity, heavy lifting, overhead reaching, and push-up-like activities should be avoided for 4-6 weeks to allow for proper healing. However, light activities such as walking or hiking are encouraged.

As with any surgical procedure, there are potential risks and complications associated with breast augmentation. These include bleeding, infection, scarring, asymmetry between breasts, changes in sensation to the breast/nipple, and potential issues with breastfeeding. Additionally, breast implants are not lifetime devices and may need to be replaced in the future. Regular monitoring of the implants, including yearly check-ups with your surgeon and adherence to FDA recommendations for MRI surveillance, is essential to ensure their longevity and detect any issues early on.
